Ownership of Damaged Property *

I certify that I am the owner of the property at which I am requesting a tree and that my property sustained tornado damage.

Permission *

I give permission to RETREET, a nonprofit organization located at 5706 E Mockingbird Ln, STE 115, Dallas, TX 75206, and its representatives, partners, and volunteers, to enter my property the limited purpose of participating in RETREET programming. Such participation may include, but is not limited to, surveying the property in order to locate an appropriate planting site, delivering supplies, digging a hole, and planting a tree.

Indemnification *

I indemnify and hold harmless RETREET, its representatives, partners, and volunteers, from any and all liability for any damage, injury, or loss to my property resulting from activities associated with RETREET programming.

Texas 811 *

I agree to allow agents of Texas 811 to enter my property in order to mark utility lines, so as to ensure the safety of the volunteers and the proper placement of the tree.

Location *

I understand that a representative of Texas Trees Foundation or one of its chosen partners will work with me to determine the best planting location for my tree in my front yard, and that the chosen location is not to be changed once decided upon and marked, unless safety dictates otherwise as determined by RETREET.

Care *

I promise to water and care for my tree after it is planted, as instructed by RETREET.

Number of Shade Trees Requested *

The trees offered are large shade trees. Use the tools below to ensure you have enough space for the number of trees you are requesting. A property of normal size may be able to accommodate 2 trees, but most will only fit one. Only corner lot residents should request 3 or 4.
